Covid-19 Reporting and forms

The CDSBEO works in collaboration with local public health units and other local health partners to ensure that schools can re-open safely. In the event of a positive COVID-19 case within a CDSBEO school, local public health authorities are responsible for determining if an outbreak exists, declaring an outbreak, and providing direction on outbreak control measures to be implemented.


An outbreak in a school is defined as two or more lab-confirmed COVID-19 cases in students and/or staff in a school with an epidemiological link, within a 14-day period, where at least one case could have reasonably acquired their infection in the school (including transportation and before or after school care).

If your child has stayed home from school based on the instructions that they should do so following the results of the COVID-19 School and Child Care Screening Tool or guidelines from the local Public Health Unit, please provide the following to the school before your child returns: Back to School Confirmation Form
